Community Sister/District Nurse

We are looking for dynamic and enthusiastic Community Sisters/District Nurses to join our Primary Care Network (PCN) teams in Crawley!

Main duties of the job
  • To have oversight of all clinical areas, facilitating junior staff to lead on specific areas.
  • Case manages a designated caseload of patients and manages / delegates the delivery of care by the integrated team.
  • To deputise for the Clinical Nurse Lead as and when necessary. Assists in the recruitment of staff, appraisal process, risk assessment process and complaints: performance management process.
  • To assist in clinical supervision / peer review including active clinical
    leadership and mentorship to junior colleagues.
  • You will be aware of the Trust financial position and the implication of service delivery. To be aware of the cost of equipment/ supplies used and to meet the needs of the patient and service in a cost effective way.

Detailed job description and main responsibilities
The emerging PCNs are groups of practices working together in collaboration with community and other health and care services. PCNs enable greater provision of proactive, personalised, coordinated and integrated health and social care.

The Community Sisters/District Nurses have overall responsibility for a designated caseload and will manage care delivery by the integrated team. The post holders will attend health and social care meetings to discuss complex patients at risk of admission and devise contingency plans.

They will also be responsible for staff management, including personal development reviews, supervision and staff development, and expected to take the lead for the team on a rota basis for handover and allocation purposes. Applicants should be experienced Community Nurses with a wide range of clinical competencies and a mentorship qualification.

Qualifications
Essential criteria
  • Registered General Nurse
  • ENB 998 and or Mentorship Qualification
  • Evidence of clinical competencies appropriate to role - Case Management; Nursing Assessment; EOLC


Desirable criteria
  • Community Specialist Qualification/District Nurse
  • Long Term condition module or relevant experience
  • Physical Assessment Module/ Clincial Decison/Case Management, llingness to undertake
  • Leadership Qualification


Experience
Essential criteria
  • Registered General Nurse 1-2 years


Desirable criteria
  • Minimum of 2 years working in the community
  • Relevant post registration clinical experience


Skills and Knowledge
Essential criteria
  • Demonstrates competency in a range of nursing skills - case management; complex health assessment; EOLC; coordination of care; wound care.
  • Ability to apply research (evidence based care) to practice
  • Advanced communication skills - SBARDD
  • Evidence of leadership skills in practice
  • Promote patient choice - patient centred holistic care
  • Knowledge of Clinical Governance, audit and clinical supervision
  • Demonstrates leadership in practice - case management for patient and understand the role
  • Reports risk and maintains accurate health records as per NMC guidance; national guidance/local trust
  • Current knowledge of NHS policies impacting on healthcare
  • Ability to demonstrate knowledge/understanding/boundaries within role
  • Good IT skills

  • Demonstrates a high level of understanding of self; able to identify areas for growth
  • Able to work in a flexible way, autonomous and respond to change
