The Beechwood Unit is excited to announce we are recruiting a Discharge Support Worker to join our Multidisciplinary In-reach Support Team (MDIST). Working as part of this team, the Discharge Support Worker will ensure that all discharges from a defined number of mental health acute beds are processed and coordinated in a way that ensures patients are placed in an environment best suited to their assessed needs and Trust resources are utilised to their maximum efficiency. The post holder holds no clinical responsibility instead focuses on the administrative coordination to support successful discharge. We are looking for an individual who is resilient, driven and passionate to provide high quality care to our patients.

Covid-19 full vaccination may be a condition of employment for this role, unless an exemption applies. The successful applicant may have contact with vulnerable service users and we will therefore seek proof of being fully vaccinated during the recruitment process.

Main duties of the job

Main responsibilities are:
  • You will work closely with the MDIS team focusing directly on the needs of patients requiring support and facilitation in order to be discharged from hospital.
  • You will develop close working relationships with ward teams providing coordinated support to the MDIST.
  • Responsible for assisting with processes and procedures to enhance patient flow as identified by the MDIST.
  • Providing follow up as appropriate for patients discharged taking action and/or escalating issues as required.

Detailed job description and main responsibilities
You will be an excellent communicator, maintain good working relationships with the team, patients, carers and service users ensuring key information is communicated in a timely, robust and efficient way.

You will work closely with ward staff, Trust teams and liaise with relevant agencies to coordinate the administrative procedures associated with discharge.

You will liaise with patients and carers and support risk identification.

You will be IT literate.
