We are looking for a motivated and experienced professionals with the relevant clinical knowledge & understanding to join our PCN Community Services Management team in EAST Sussex as a Clinical Services Manager (CSM). Covering the Brighton & Hove and High Weald Lewes and Havens area

The Clinical Services Manager (CSM) role is an ideal step up from a Team Lead role and is an opportunity for you to support our Primary Care Network (PCN) Teams using your experience & excellent communication skills.

The role will provide leadership alongside other CSM colleagues to our dynamic community-based teams. These dedicated teams work hard to deliver treatment, prevent unnecessary admissions to hospital and support discharge by providing nursing & Therapy care to the local population enabling them to remain in their homes.

As one of our CSMs, you will enable these crucial services to do their amazing work through your supportive leadership, service development, and service integration to improve patient pathways and experience.

Your pivotal role is to empower your teams and support multi agency collaboration within our system-wide approach to maintain and promote patient care & experience.

Main duties of the job

As a CSM, you will lead all aspects of the services provided by your PCN Teams ensuring that they meet their operational and quality objectives.

You will lead on the planning and delivery of these objectives in line with the Trust's annual business planning cycle, and actively monitor delivery throughout the year coordinating resources across teams as necessary.

Detailed job description and main responsibilities
The Clinical Services Manager/Therapies Manager/Matron are responsible and accountable for the delivery of a defined service or function. The post- holder is responsible for managing all aspects of the service ensuring that the service meets its operational and quality objectives and targets.
The Clinical Services Manager/Therapies Manager/Matron will lead on the

Qualifications
Essential criteria
  • First Level Registered Nurse/or Relevant professional degree or diploma
  • Leadership Qualification


Desirable criteria
  • Continuing Professional Development and learning


Experience
Essential criteria
  • leadership role including contributing to operational management of services
  • Experinece of MDT and multiagency working


Skills and Knowledge
Essential criteria
  • Evidence of ability to monitor and maintain standards of care
  • Evidence of monitoring and analysis of KPIs
