If the answer is YES then the post of Clinical Nurse Lead is waiting for you.

The Brighton and Hove Integrated Primary Care team is based at Conway Court in Hove and provides domiciliary nursing and therapy to the population cared for by Hove's PCNs.

Main duties of the job

The Clinical Nurse Lead is an exciting and autonomous role supported by the Operational Team Lead and the Practice Development Nurses. It plays a key role in supporting staff to develop and maintain their clinical competency, ensuring teams deliver the right care at the right time to the individuals on the community nursing caseload.

The successful candidate will hold a CSP or an equivalent qualification. They should be able to demonstrate excellent communication and negotiation skills, as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the PCN. The Clinical Nurse Lead will be required to support the community nurses and the PCN partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.

Detailed job description and main responsibilities
The post holder will be required to lead the provision of a clinically effective, cost
efficient service and clinically lead a team of nurses. They will be responsible
for the development and delivery of a consistently high quality service,
promoting the team as a positive place for staff experience and development.
The post holder will demonstrate at all times a comprehensive knowledge
base and promote excellence in nursing practice in a dynamic healthcare
environment. As an active part of the team, the post holder is expected to
provide direct patient care when necessary, acting as an expert practitioner
and a role model to the whole team. It is essential that they can use initiative
and work without direct supervision. They will co-ordinate services to promote
optimum health, self-management and effective liaison between agencies.

Qualifications
Essential criteria
  • RGN first level nurse
  • Post graduate level degree or equivalent
  • Advanced Physical Assessment course Masters Module
  • Non Medical Prescriber or willing to work towards


Desirable criteria
  • Masters qualification
  • Mentoring Course or equivalent
  • Community Nursing Qualification


Experience
Essential criteria
  • Extensive clinical experience five years
  • Case management experience
  • Evidence of up-to-date based knowledge and skills, particularly in Long term Condition issues
  • Evidence of ability to maintain and monitor high standards of care
  • Evidence of continuing professional development
  • Eighteen months post graduate experience

  • Evidence of ability to direct and co-ordinate programmes of care working autonomously and collaboratively

  • Evidence of advanced communication skills, able to understand blocks to communication and ways of developing strategies to overcome these.

  • IT literate or willing to achieve within an agreed period.

  • Ability to travel between sites, car owner/driver with access to a vehicle for work purposes
  • Demonstrate a high level of understanding of self and able to identify personal limitation and shows openness to address them
