Job overview

We have an exciting opportunity for experienced Advanced Nurse Practitioner's to join the Intermediate Care Units at Crawley Hospital and Horizon Unit within Horsham Hospital, part of Sussex Community Foundation Trust (SCFT).

You will work as part of a team providing high-quality care on one of our three rehabilitation units, one of which incorporates ten stroke rehabilitation beds. We offer our patients a period of rehabilitation prior to going home or placement, as well as end of life care. This makes our environment both varied and interesting which will allow you to utilise and develop all your skills.

Main duties of the job

We work as a multidisciplinary team (MDT) which includes our Doctor, Therapists, Social Workers and our ANP colleagues. Although you work as part of the MDT you will need to be able to work autonomously at times.

You will have completed and be experienced in prescribing and have advanced assessment skills helping you to provide advanced clinical nursing knowledge, expertise, and leadership to the Intermediate Care Unit. At times you will work on other units providing cross cover for colleagues as required.

Using your skills and competencies as an Advanced Nurse Practitioner such as history-taking, physical assessment, and prescribing, you will assess, diagnose, and prescribe for patients with complex and/or urgent health needs and managing the episode of care as appropriate, liaising with acute and community colleagues as appropriate.

Qualifications/Experience
Essential criteria
  • RN
  • Physical Assessment module at degree level
  • Independent Nurse prescriber with experince
  • Teaching/ Mentoring experience and / or qualification


Expereince
Essential criteria
  • Minimum of 5 years post registration experience to include some hospital experience
  • Evidence of appropriate continuing professional development activity to maintain up to date knowledge and on-going competence
  • Evidence of working autonomously and as part of a team
  • Experience in Management of long term conditions eg. Asthma, COPD, diabetes and CHD
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice


Desirable criteria
  • Interpreting and implementing local and national policy agenda for health


Skills & Knowledge
Essential criteria
  • Understanding and knowledge of policy development related to the delivery of
  • Knowledge of national standards that inform practice (eg. National Service
  • Understanding of their accountability arising from the NMC code of Professional
