We are seeking Discharge Support Workers across four wards at the Langley Green Hospital and will consider full or part time applicants.

We are excited to announce the establishment of a new Multidisciplinary In-reach Support Team (MDIST) are Discharge Support Workers will play an integral role in supporting successful discharge. You will ensure all discharges from a defined number of mental health acute beds are processed and coordinated in a way that ensures patients are placed in an environment best suited to their assessed needs and Trust resources are utilised to their maximum efficiency. The post holder holds no clinical responsibility instead focuses on the administrative coordination to support successful discharge. We are looking for individuals who are resilient, driven and passionate to provide high quality care to our patients.

Main duties of the job

• You will work closely with the MDIS Team focusing directly on the needs of patients requiring support and facilitation in order to be discharged from hospital.
• You will develop close working relationships with ward teams providing coordinated support to the MDIST.
• Responsible for assisting with processes and procedures to enhance patient flow as identified by the MDIST.
• Providing follow up as appropriate for patients discharged taking action and/or escalating issues as required.

Langley Green is a state-of-the-art hospital for people with acute mental health problems. The hospital has two acute care working age wards, one integrated working age/older adults ward, and one psychiatric intensive care unit (PICU).

Detailed job description and main responsibilities
You will be an excellent communicator, maintain good working relationships with the team, patients, carers and service users ensuring key information is communicated in a timely, robust and efficient way.

You will work closely with ward staff, Trust teams and liaise with relevant agencies to coordinate the administrative procedures associated with discharge.

You will liaise with patients and carers and support risk identification. You will be IT literate.
