Job overview

It's an exicinting time to join our Worthing Responsive Services team as we expand our current urgent care work.

We are looking for a wide variety of posts including this role in which you will provide nursing assessment and intervention to patients within their own homes. The main reasons for referral are to avoid unnecessary admissions to hospital and to support patients who are discharged from hospital under the Discharge to Assess pathway.

There are full time and part time posts available which allows you to have greater flexibility in working hours.

Main duties of the job

In this role you will deliver multi professional high quality care within the community setting working with patients towards rehabilitation, self management and independence throughout the continuum of care.

You will manage the day to day, clinical needs of an identified caseload determined by the caseload coordinator. This involves undertaking first visits and holistic assessment of needs, and on going management of personalised care within the scope of practice working under
the guidance of the caseload coordinator and the Clinical Lead.

Detailed job description and main responsibilities
Our Responsive Services are made up of multidisciplinary teams providing a seven day service.

The post holder will need to have experience of working within a variety of clinical settings, ideally with the community. Car drivers/owners are essential due to the geographic area covered by the team.

  • RN Qualification degree or diploma

  • Mentorship qualification

  • Experience in a community setting is desirable
  • 1 year post registration experience


Skills and Knowledge
Essential criteria
  • Good numeracy and literacy skills
  • Good communication and negotiation skills
  • Demonstrates competency in a range of clinical skills e.g. wound management, catheterisation skills, medicines management and administration, palliative and end of life care including management of syringe drivers.
  • Promote independence and choice in undertaking patient care
  • IT literate or willing to achieve within an agreed period
  • Knowledge of Clinical Governance, clinical audit and clinical supervision, Professional codes of Conduct
  • Ability to demonstrate knowledge/ understanding of boundaries of role


Other requirments
Essential criteria
  • Ability to travel between sites, car owner/driver, with access to a vehicle for work purposes
  • Ability to work flexibly, including participating in an integrated service covering seven day working from 08.00 to 20.0o
