Job overview

We have an exciting opportunity for an enthusiastic and conscientious practitioner to join our 0-5 team based in Rustingston. Our service is open Monday to Friday 9-5. Previous experience of working in the community is not essential.

Are you a qualified or about to qualify (child, adult, mental health or learning disability) nurse or a midwife with a passion for public health and improving outcomes for children in West Sussex? Would you like to work for a friendly and passionate Health visitor team who make a difference to children's lives every day?

Alongside a skill mix team of health visitors and nursery nurses, you will work in collaboration with families; providing universal and targeted early interventions to ensure every child is offered the best start in life.

We are looking for a compassionate individual who can demonstrate excellent communication skills and is able to work in partnership with multi-agency and inter-professional colleagues to support the delivery of the Healthy Child Programme (HCP).

This is a permanent position but may also be an excellent opportunity to gain experience for an individual interested in undertaking a Specialist Community Public Health course in health visiting or school nursing in the future. A programme of support is being developed to support internal applicants interested in applying for a funded degree to become a health visitor or school nurse.

Main duties of the job
  • Managing a clinical caseload
  • Undertaking holistic assessments, identifying health needs, parenting capacity and family/environmental factors
  • Implementing and evaluating programmes of care
  • Delivering evidence based health promotion advice
  • Undertaking targeted support in line with our commissioned service
  • Inter-professional working to deliver the healthy child programme and safeguard children

West Sussex HCP have achieved full UNICEF UK's Baby Friendly accreditation and hold the UNICEF Gold Award for embedding Baby Friendly care in our workplace.
