37 hours per week - Chanctonbury - £24,000 pa

18 hours per week - Brighton - £11,675 pa

18 hours per week - Bognor Regis - £11,675 pa

37 hours per week  - Arun AIC - £24,000 pa

The Role

Due to the expansion of our Social Prescribing Service, we have some exciting opportunities for new Social Prescribers to join our amazing and dedicated team in various locations across West Sussex. You will be a key member of the Primary Care Network (PCN) multi-disciplinary team and will provide support to patients, empowering them to take control of their health and well-being. The support you give will focus on what matters to the patient, taking a holistic approach to each individuals health and well-being. You will be connecting patients to community groups and services for practical and emotional support.

Although AUKWSBH supports people over the age of 50, our Social Prescribers support all patients from the age of 18. You will be helping patients set goals, achieve personal growth, and behaviour modification as well as building resilience and social connectedness, reduce inequalities and help patients to have greater control over their health and lives.

If you have a good knowledge of local community activities and the initiative to find out more about the range of non-medical options available, then being a Social Prescriber could be the role for you. You will need to be well organised, able to manage a caseload, have good IT skills and be able to keep accurate records.

These posts are exciting opportunities, giving the successful candidates the opportunity to deliver a service which will improve people’s lives and make a real difference.

All roles offer hybrid working, with a mixture of working in GP Surgeries, office and homeworking. Please indicate on your application which area you are applying for

Bognor Regis - You will need to cover Monday and Friday and other days are flexible and can be discussed at interview.

Brighton - working days can be discussed at interview.

Arun and Chanctonbury - both full time roles but will offer flexible working patterns which can be discussed at interview.

A driving licence is required for the role as it may involve some travel.
