Job overview

High Weald Lewes and Havens SOAMHS team is seeking to recruit a specialist nurse practitioner to work across both Newhaven and Uckfield sites. The role will include weekend working to support our Intensive Support Team as well leading and developing clinical practice within the wider SOAMHS team. They will work alongside the Team Leader to support operational and clinical performance management including supervision and management of staff.

Main duties of the job

The post holder will provide specialist mental health assessments and nursing interventions to patients referred to our service, and will work alongside nursing and MDT colleagues to support and develop clinical practice within the SOAMHS team, supporting older people with both functional mental health issues and those with dementia.
The post holder will be flexible in their approach, providing patient centred care and working closely with families/carers, the acute trust and our multi-disciplinary team to ensure the best outcomes for every person referred to our service, including appropriate onward referral to other services and support networks.

To provide a specialist assessment, including risk assessment that is the basis for clinical decisions and on-going care provision for all adults, including those with functional mental illness and dementia.

Person specification

Qualifications
Essential criteria

• Registered Nurse with current registration (mental health / LD/ Adult) appropriate to the job role
• Degree/diploma level supplemented by specialist qualification, training, experience, courses to masters level equivalent relevant to the care group
• Evidence of post registration training in relevant service area
• Completion of Mentorship Course/ENB equivalent

Knowledge/Experience
Essential criteria

• Extensive post qualifying experience in relevant service area
• Experience of supervising staff and students
• Able to manage a caseload of patients with complex mental health needs
• Comprehensive knowledge of interventions specific to service area
• Awareness of Trust policies and service specification
