We are looking to recruit skilled and highly motivated practitioners to join us as Band 6 Urgent Care Practitioners in the CRHTT, working closely with the acute hospital and community teams to support people in a mental health crisis. CRHT are able to offer all the benefits of a hospital admission whilst remaining at home, including home visits from our experienced multi- disciplinary team. We are seeking to recruit a full time shift co- ordinator to monitor and to manage referrals to the crisis team. This role is complex and requires a strong understanding of policies and protocols relating to Urgent Care. We are committed to a focused recovery model. This post may suit someone who is keen to undertake a mixed shift pattern of 7:00am to 3:00pm and 1:00pm to 9:00pm four days per week over a 7 day period.

Main duties of the job

You will be an excellent communicator, demonstrating empathy and facilitating an inclusive and collaborative environment and assessment to those service users accessing CRHT services with a particular focus on:
  • Utilising your excellent clinical skills and judgement to assess individual needs and risks
  • Demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and awareness of alternatives to hospital admission.
  • Work compassionately and creatively to identify therapeutic and evidence-based outcomes and care plans for people who are experiencing a crisis in their mental health
  • Work closely with carers, families and partner agencies to facilitate and promote the safety and the well-being of service users and their families/carers.
  • work collaboratively across the urgent care services being responsible for referrals accepted into the team.

Detailed job description and main responsibilities
This is an exciting and dynamic role that offers the opportunity for development and implementation of high-quality services. Previous experience of working in acute services or home treatment teams is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.

This service is integral to our Urgent Care Pathway and ensures people presenting to acute mental health services receive a compassionate, person- centred assessment and timely care plan.

This role supports the work of CRHT and will necessitate frequent contact with Assessment and Treatment teams and GP's and other agencies to discuss, accept and plan treatment for appropriate referrals.

As such, this post has the following shift pattern : 7:00am to 3:00pm and 1:00pm to 9:00pm 7 days per week.

Qualifications
Essential criteria
  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development

Knowledge/Experience
Essential criteria
  • Significant post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community
