Job summary We have a great opportunity for a full-time band 3 community support worker to work in the Dementia & Older People's Mental Health Team based in Horsham area and will be covering Crawley/Horsham and Chanctanbury in West Sussex. DOPMHS operates a pattern, Monday to Fridays, 9am - 5pm. this is an exciting post working with people with dementia and mental health needs. The ability to travel is essential in this role, driving is also essential. Main duties of the job The role of this post is to actively engage those requiring support with their mental health, co working with lead practitioners within an MDT set up. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work. You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.
