Job Details
Job summary The team manages all incoming referrals from GPs, mental health services, Adult Social Care, third sector organisations and takes part in the triage hub with these services to ensure patients get treatment from the service that best meets their needs. Following initial referral, triage staff conduct risk screening over the phone with patients and make decisions about what happens next. In addition, the newly extended team will now be conducting all assessments (except those requiring medic/OT/Psychology). Parking costs are paid. Main duties of the job To be a successful in this post you will be:
- A Registered Mental Health Nurse/OT/Social Worker
- Have good assessment and decision-making skills
- Excellent at forming relationships with our partners
- Keen to embrace new ways of working and enthusiastic about improving clinical practice
- Experience working with adults with severe mental illness
- Experience working in a community team
