Job summary
There are 2 full-time equivalent roles available, one based in Chichester and the other in Worthing.
The In-Reach Practitioner will support the System Capacity and Flow Manager in managing patient flow from acute providers to and across SCFT Intermediate Care Units
To work collaboratively with external stakeholders and partner organizations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilized and patient flow is maximized
Main duties of the job
To communicate with clinical and managerial teams within Sussex Community NHS Foundation Trust in the provision of updates and assurance about patient flow and common themes and incidents relating to admission and discharge
To communicate with community teams to establish their capacity to manage patient flow/patient transfers and escalate as required, using SHREWD as a capacity reporting tool
To communicate highly complex and sensitive patient information in a timely and efficient manne
During periods of high activity across the health and social care systems the post holder will manage the associated demands and events to the best of their ability and seek advice if required from their line manager
