Job summary Due to ongoing funding being available an opportunity has arisen for us to continue the existing mental health provision available within the Rough Sleeping Initiative (RSI). We are looking to appoint a Band 6 Occupational Therapist on a fixed term to work within the team, mainly covering west side of the county until end of March 2025. The RSI has been operating across East Sussex since Oct '18 with specialists from the main statutory services. The Multidisciplinary Team including SPFT (senior nurse, two band 6 Occupational Therapist and two band 4s), Adult social care, CGL, Probation, local Authorities. The service is supported by Sanctuary Housing and Southdown Housing providing housing first support and has had marked results in terms of rough sleeper engagement, crisis intervention and tenancy sustainment due to the multi- approach, all offering a bespoke wrap around service and support to a complex group of people. Main duties of the job
  • Integral part of a small mental health team (mental health nurse, occupational therapists and two band 4 support workers) assessing and supporting a disadvantaged cohort with their mental health needs. Also working alongside other professionals including Practice nurses, social workers, substance misuse specialists, Probation officers, as well as housing specialists from each local borough, Outreach workers and housing support workers within the team.
  • Occupational Therapy assessment, planning and support for identified clients mainly west of the county (Eastbourne, Lewes and Weald)
  • Graded Activity Plans to ensure achievable goals and sustained engagement
  • Developing individual programmes which feed into wider holistic care plans.
  • OT support focus will be to establish routines, healthy habits, develop daily living tasks and explore interests, activities and training opportunities.
  • Supporting TIC approaches to the management of anxiety, depression and chaotic behaviours.
  • Working closely with members of the multi-disciplinary team and SPFT colleagues within the trust to help prevent relapse and develop coping techniques and self-management of conditions.
  • Close working with East Sussex Rehabilitation Pathway colleagues.
  • Attending team and other relevant meetings.
