Job summary

The Coastal West Sussex and Mid-Sussex Diabetes Specialist nursing teams are looking for a dynamic and motivated individual to join them. You must be passionate about providing excellent diabetes care, improving outcomes, service development and supporting staff to be the best that they can be.

The role is focused on supporting the operational delivery of the service and clinically assessing and devising care plans for people with diabetes. Educating individuals and health care professionals on treatment options and lifestyle changes. A large part of the role is working alongside primary care colleagues to improve outcomes by imparting knowledge and skills to the patient, their family/carers, practice nurse and GP.

Main duties of the job

The successful candidate must have and be able to demonstrate comprehensive diabetes knowledge and act as an autonomous decision maker. You will be expected to use your skills to teach and support programmes of care to individuals, primary care colleagues and act as a clinical resource for other health care professionals.

You will be operationally accountable for the team in Coastal West Sussex and Mid-Sussex to deliver care in line with current evidence based practice and local/national guidelines. Therefore the post holder will require strong leadership qualities, excellent communication skills and an ability to prioritise their workload.

Experience of managing a team is desirable but not essential.
