Job summary

It's an exciting time for our Urgent Community Response (UCR) Service. We are expanding our teams to include the ability to provide virtual ward capacity whilst continuing to build on our successful UCR model.

UCR services work with patients in their homes across West Sussex communities to avoid hospital admissions, support those being discharged home and provide virtual ward support using remote monitoring technology and enhanced medical oversight.

We are looking for an Operational team Lead to join us, so if you're a dynamic and patient focused individual, with a passion for leading and supporting teams to deliver excellent care at the heart of the community, we'd love to hear from you.

We are a multidisciplinary team working flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm. We deliver:
  • A rapid response to people in the community who are experiencing acute episodes of ill-health.
  • Virtual ward admission utilising remote monitoring technology and enhanced medical oversight to provide a safe alternative to hospital admission.
  • A supported discharge service to facilitate timely hospital discharges.
  • Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

Main duties of the job

In this role, you will provide operational leadership, ensuring effective governance and support for staff working within the service. Operational duties will include overseeing the service budget, safe rostering of your staff to cover service demand and performing line management duties in line with Trust policy.

Working with the clinical leads, you will ensure the clinical effectiveness of the service and the delivery of an excellent patient experience. This will include reporting and managing incidents, and analysing data to monitor performance. You will develop key partnerships with colleagues to ensure effective working of the whole system.

You will have varied communication styles for strong leadership and multidisciplinary working. It will be your responsibility to ensure staff feel informed and to represent your team at a senior management level. You will be report directly to the Clinical Service Manager who will support you in your role.

Your experience of confident leadership while promoting a team philosophy of supporting reablement focused care will help you excel in this role. We will support you to develop your leadership skills through internal and external programmes.
