Job summary

It's an exciting time for our Urgent Community Response (UCR) Service. We are recruiting to our teams who will work with patients in their homes across the West Sussex communities to avoid hospital admissions and support those being discharged home.

We are looking for Staff Nurses to join us, so if you're a motivated and enthusiastic nurse looking to be part of a supportive team and use your skills to provide excellent urgent community care, we'd love to hear from you.

This is an amazing opportunity to become a member of the team as a staff nurse working together to help patients stay in their homes, making sure they avoid unnecessary hospital admissions and supporting them through the first few critical days after they leave hospital. The focus of this role is delivering high quality care in the community, working with patients across HWLH towards re-enablement, self-management, and independence throughout the continuum of care. You will do this by delivering holistic assessments, planning, implementing, and reviewing care provision.

We are a multidisciplinary team working flexibly to provide a seven-day service, 365 days a year, 8am and 8pm. We deliver:
  • A rapid response to people in the community who are experiencing acute episodes of ill-health.
  • A supported discharge service to facilitate timely hospital discharges.
  • Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

Main duties of the job

In this role, you will be part of a team of colleagues supporting evidence-based care, facilitating staff learning and development. You will provide nursing input for patients including medication management, wound care, catheter care and identifying deteriorating patients. This post is an ideal opportunity for a nurse who is keen to develop both their holistic assessment skills and knowledge of urgent community response.

If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme, through protected time and regular clinical supervision with dedicated registered professional support.

You will need to able to drive with a full UK license due to the geographic area covered by the team.

The team works closely with other services within Sussex Community NHS Foundation Trust, as well as our acute and community partners, GPs, South East Coast Ambulance Service (SECAmb), adult social care and voluntary and third sector organisations.

Qualifications and Professional Registration

Essential
  • NMC registered qualification
  • ENB 998 or equivalent or work towards
  • Evidence of a range of clinical competencies

Experience

Essential
  • Varied experiences in a variety of clinical settings
  • Community experience
  • Evidence of a range of clinical competencies appropriate for the role

Desirable
  • Experience of working with the ageing population
  • Experience of EOL patients and EOL care

Skills and Knowledge

Essential
  • Knowledge of Clinical Governance, clinical audit and clinical supervision, Professional codes of Conduct
  • Demonstrates competency in a range of clinical skills e.g. wound management, catheterisation skills, medicines management and administration, palliative and end of life care including management of syringe drivers
  • Knowledge of current local policies informing community health and social care services
  • A clear understanding of the role of the Community Staff Nurse
  • Is able to demonstrate the application of research based practice
  • Promote independence and choice in undertaking patient care
  • IT literate or willing to achieve within an agreed period
  • Ability to demonstrate knowledge/ understanding of boundaries of role Practice supervisor

Other Requirements

  • Car driver (Valid Driving License for use in the UK) and Access to a vehicle for work purposes
