The Team work collectively with other professionals and providers to meet the needs and outcomes for children and young peoples short break care plans.

Following a child and family assessment to assess the needs of our children and young people, a short break plan is identified to support the child to access a short break and support the family.

In respect of the role the candidate will be able to manage a caseload of short break cases for Children with Disabilities under the guidance of the Senior Social Worker and Team Manager. This will involve a short break care plan to be kept under review and ensure that the direct payments or alternative local authority short breaks meet the child's assessed needs.

The candidate will actively work as part of the whole system, and in line with associated policies and procedures of the service area with the aim of achieving the best possible outcomes for children, young people and their families.

The role will also require a flexible and responsive approach to identified needs.

The candidate will be expected to undertake direct work on occasions with families as part of the child's plan as directed by the Senior Social Worker or Team Manager.
