Impetus (EPiC Community Navigators Project)

Impetus delivers a range of services aimed at improving the well-being and quality of life of vulnerable adults across the City. Our service users include people with learning disabilities, people with mental health issues, older people, people with physical disabilities and people with autistic spectrum conditions. EPiC (Extended Primary Integrated Care) is a new and innovative project that is nationally recognised and will initially run as a pilot from October 2014 to May 2015. We are currently recruiting Community Navigators to meet individuals within their GP surgeries, identify their community support needs and signpost them to appropriate sources of social support and other non-medical services within their neighbourhood and beyond.
